A middle age male presented with complaints of pain in right hypochondrium with intermittent episodes of nausea and vomiting.

Findings:

Few echogenic calculi with post-acoustic shadowing were noted in cystic duct. There was resultant intrahepatic biliary radical dilatation. There was dilatation of common bile duct as well.

Key-points:

On ultrasound;

• intrahepatic bile ducts are considered dilated when,
  • >2 mm
  • >40% of adjacent portal vein
• extrahepatic bile ducts (common hepatic duct and common bile duct) considered dilated when,
  • usually measured in the proximal duct, near the proper hepatic artery
  • diameter measured from inner wall to inner wall
  • >6 mm +1 mm per decade above 60 years of age
  • >10 mm post-cholecystectomy ²
